Why so many factory defects?

It seems like SG-1 is trying to set a record for show with the DVD sets with the most factory defects ever. I've heard loads of horror stories about people who couldn't get disc 5 on their Season 7 set to work. I also have friends who keep running into problems, first with disc 3 on Season 4, then on disc 5 on Season 9. And I keep hearing about other people that suffer similar problems. What's the deal?
 
I've got almost 2 of each season (darn switch to slim packs) and the only issue I've had was with the Threads episode version - and they sent me a replacement.

If there's a problem I'm surprised I escaped for once.
 
I've never had a problem outside of that stupid Threads debacle, but I have heard horror stories about those season seven DVDs (that's what MGM gets for cramming five episodes, all with commentary tracks and the special features onto one disc).
 
I had the issue where Disc 5 of Season 7 would play the first few seconds of "Lost City" and then black out. I also had the "Threads" Season 8 issue. But neither of them were real debacles in any way (for me) - they were easy enough to replace with working discs and correct versions. I hear about glitches and problems like this all the time, but I'll admit that it is a bit unusual for it to happen numerous times to one series.
 
The only DVD issues I've experianced are:

-SG-1 S8, the infamous Threads incident.

-Atlantis S1, the fifth disc was a copy of disc 1. Indeed, all the sets that shipped to Canada were like this.
