The comic is adding Hellblazer to it's title.Sounds like a good plan, cast, crew and sets are already in place, a move to syfy might be quite a bit cheaper than a completely original project... As for the name change... no bloody chance in hell.
Perhaps a scenario like auntiehill's might happen, But a complete name change and at the same time moving the show to another network... too high a risk to lose a chunk of the audience.

Well, they've already failed to capture a large general audience, which is why its odds of survival on NBC are low. So the kind of casual viewers who'd lose track of the show through a network/title change aren't watching it anyway. But it does have a loyal cult audience, the kind of audience that would follow it wherever it went. And a cable network is a better fit for cult/niche shows.
Of course it wouldn't be a guarantee for success, but nothing ever is. Everything in TV is a gamble. But taking a chance on keeping the show alive has got to be a better option than just cancelling it.

That was the name of the original comic for 25 years. So it's more like "restoring" than "adding."

I have never read the comics or seen the film so my knowledge of Constantine comes only from the show but would have loved to know what is Manny's plan for Constantine.
