The "phased" Romulan was sitting in a chair...
Obviously the chair was gravity plated.

While they may not have looked too scary, I think the parasites from Operation Annihilate are by far the scariest creatures in Trek.
To me the creepiest/grossest creature was...
This is the only episode of TOS that actually freaked me out as a child. Though the creatures themselves don't bother me now as an adult, I recall "how" they made me feel as a child & it's that feeling I don't like recalling.
Romulans never invented seat belts...Obviously the chair was gravity plated.

That was supposed to be the Borg, but it was too expensive to show a ship full of insectoids, so they changed them to humanoid cyborgs.I recently rewatched this episode as a result of this thread. These creatures are actually not unlike the Borg in certain respects- the big scare of the TNG era:
1) They force other species to work for them
2) They do so by 'infecting' and kidnapping their nervous system which they can do within seconds
3) They draw their strength from their collective state and are able to resist phaser fire this way- specimens are more like cells of a larger organism than individuals.
We call those “hide-behinds” in my neck of the woods.@)Yes! The featureless hologram shadow terrified me.
It's amazing to realize this was an actress, think her name was Meg Wylie? The heavy makeup and latex created a virtually genderless character, and the astounding thig was how perfectly she acted the part...

When I watched this episode as a kid for the first time, it never occurred to me that these were women.I don't know. To me, her character still looks like a woman, not a very attractive one though.
I see nothing.
I see nothing.
Neither do I, only an icon of a missing image.
The creatures from TNG's "Identity Crisis" get my vote; their method of reproduction and how they rapidly overwrite their host's sentience/sapience with pure instinct are terrifying.
