I realise this might be a long shot, but I've been trying to track down where this particular alien head piece may have been used. It appears in the Deep Space Nine magazine vol 3 from 1993 in a photo with an interview with Michael Westmore. It's an orange/pinkish head piece with multiple eyes. Other pieces pictured include Morn, Ferengies, and other alien species I have been able to spot in Ds9, but not the multi eyed alien. I'm hoping someone knows which episode it appears in that I may have missed.

I know exactly the one you mean, though I don't have a picture to share. It's a featureless alien head except for eight evenly spaced eyes.

That pic was posted by Jörg Hillebrand, asking if anyone knew anything about it. And he generally knows all this kind of stuff, so not a good sign. Just an unused design, I'll wager.

Exactly how it looks. I can't get your link to work at the moment, nor am I sure how to load a photo of it here, but that sounds like the right one. It'd be a great shame for this amount of work to end up on the cutting room floor back in the day!

The only place where I have been able to find it was in the 1996 game "Star Trek: Klingon". I am not aware of any appearance in canon Star Trek.

I have always been fascinated by this alien make-up and was happy when I finally found it in "Star Trek: Klingon" - but that game is from 1996 and the magazine with the photo is from 1993, so that alien must have been created for something else and then was re-used a few years later in the game. Maybe that alien was just a concept or it was used in a deleted scene. We'll never know unless we ask Michael Westmore.
