Looking for Dedication Plaque template

[Cobalt Frost]

Specifically, an Adobe Illustrator (or possibly InDesign) template. I have one for Photoshop, but it's not quite as versatile as I'd like, and I'm much more well-versed in Illustrator. I have been digging around the net without much success, so I figured I'd ask here. Thanks in advance!

Sure, lay it on me! I found another PS template the other night, but I still want one for Illustrator as the PS template pixellates too much for my taste.
